Of course there are some commercial tools like external content lockers available for this task.

But the easiest way to hide the referrer is to load a new page with a "meta" tag like this:

Code:
<meta http-equiv="refresh" content="5; URL=http://www.domain.com/">

And the destination page then calls the content locker or whatever page you like.

What do you mean by external content lockers? Could you please elaborate on that? Thanks!

I had a look inside last year, but never used one of those in a production environment.

I'm using my own content locker to hide referrers, display offers in a special sorting, split test offers of several networks and arrange some special things...

I see. I've read some reviews on both external content lockers and none of them are mobile responsive yet, so I'm really turned off. :/
Anyway, what about on a 301 redirect? For example:
My site -> new domain (301 redirect) -> Offer page

If I can't edit the offer page, how can I erase the referrer when using a 301 redirect? Thanks!

Therefor I'm using my own content locker. First it analyzes the source country, then it analyzes the device used (mobile phone, tablet, desktop). And then after some more preparations the offers are displayed.

The domain you're redirecting with 301 to is then visible as a referrer to your CPA network(s):

yourhiddendomain.com -> (redirected with 301) yourdomain.com -> CPA network

So yourdomain.com is the referrer.

The best and free way to do is using the meta refresh property as the user "BlackHatSEO" mentioned.

Even in 301 redirect, I heavily doubt that you would not be able to hide the referrer.

I am not sure whether Bit.ly uses 301 or not but even shortening your link with it and visiting it, you will see that it will reveal the original referrer and would not mask it.

The best solution is to use that meta refresh property and set the "number of seconds" to 0 or 1 and your users won't feel or experience any delays as well.

Hope that helps.

What I do is redirect my traffic from a source to a blogspot domain and use what Blackhatseo told.
