I am looking for the code that makes someone share the website link and have a unique invite link that people click on which then takes them after they gather 5 other people to the survey link.

That website is down from the amount of traffic lol! Seriously now, I am certain I saw a script around here for exactly what you want, pretty basic but with some editing you could make it work. If you can't find it, hire someone to do it, it can't be that expensive. All you need is a database for the IP's of the visitors (or use a cookie) to prevent people from clicking their own link, and a simple code to generate a unique link for each visitor to share. Than the code checks if the number of visits brought to his link is equal to the one you set (5 for example) and the survey link unlocks. Add some nice graphics and you're done!
Be sure you can handle the traffic, though!
